We have a special group price of just $8 per ticket (normally $20 each). For a special treat the Faith Lutheran School Choir will be signing the National Anthem before the game! IT'S TIME TO ENROLL We are pleased to announce that the tuition prices for the 2012-2013 school year will remain the same as this year! We will be offering the Early-Bird discount for enrollment fees. If you pay your enrollment fees by March 1, you will receive a $50 enrollment fee discount! If you pay your fees by April 1, you will receive a $25 discount. After April 1, the price will go back up to $200. So, if you pay fees by March 1, the fee will be $150, if you pay by April 1, the fee will be $175. Save yourself some money and enroll early!!!! |

If a Physician Assistant (PA) completes the Child Health Assessment, the signature of the licensed Physician authorizing the PA is to be included at the bottom of this form. |
